There are two ways to get Wrathful Hearts in Diablo 4:
- Crafting
You can craft Wrathful Hearts by combining 40 Brutal Malignant Ichor, 40 Devious Malignant Ichor, and 40 Vicious Malignant Ichor at Cormond’s Workbench. This will create an Uncertain Heart, which has a chance to be upgraded to a Wrathful Heart.
- Drops
Wrathful Hearts can also be dropped by enemies in the Malignant Tunnels. These tunnels are located in the Scouring Wastes, and they are only accessible to seasonal characters.

- Craft Uncertain Hearts
The best way to farm Wrathful Hearts is to craft Uncertain Hearts. This will give you a chance to get a Wrathful Heart every time, even if it’s not a guaranteed drop.
